We are a private billing practice, meaning that
• all concession/healthcare card holders, DVA patients, children under 16 and over the age of 65 are bulk billed Monday to Friday from 8 am to 5 pm
• all concession/healthcare card holders over the age of 16 and under the age of 65 will be charged a low out of pocket fee for their appointments
• Before 8 am and after 5 pm Monday and Friday there is PRIVATE BILLING for all patients (including children and concession card holders).
• Weekends and Public Holidays fees differ Saturday (8.00am-2.00pm) & Sunday (8.00am-2.00pm) PRIVATE BILLING for all patients (including children and concession card holders).
• Dr Singh is a 100% PRIVATE BILLING DOCTOR including adults , children and concession card holders

Brief Consultation: $65.00 ( Concession Rate : $ 40.00)
Standard Consultation: $86.00 ( Concession Rate : $60.00)
Long Consultation: $145.00 ( Concession Rate : $110.00)
Out of Hours Fees before 8am and after 5 pm , Saturdays after 1pm , Sundays and Public Holidays

Walk- Ins
We normally require people to make a booking but we can sometimes help if you walk into the clinic. Priority is given to people who made a booking.PLEASE NOTE ALL WALK INS WILL INCUR A PRIVATE FEE.
Home Visits
If your a regular patient and can't travel to the medical centre , your GP may be able to visit you at your home. This is at the discretion of your GP.
Pre-employment Medicals and Driving License Medicals are not covered by Medicare and have private fees attached.
There will be a Gap Fee for skin check and skin procedures.
Certain surgical procedures also incur additional costs. Please ask the doctor or nurse prior to any procedure if any out of pocket expenses will be incurred.
All accounts will be payable on the day.
Reception will be able to submit your claim to Medicare on your behalf after payment of your account, provided you bring your CURRENT Medicare Card and Debit Card.

Your doctor or the practice nurse will attempt to contact you immediately via the contact details supplied.
If our attempts at contact are unsuccessful, a letter will be forwarded to your address to contact us urgently for an appointment.
Our nurse will contact you via phone to advise that you need to make a non-urgent appointment to see your doctor.
If our attempts to contact you are unsuccessful, a letter will be forwarded to your address to contact us for a non-urgent appointment.
Please note: if your results are normal we will not contact you.
You are welcome to phone us to check if your results have been returned to us, but you will have to make an appointment to discuss these results.
At no time are results provided to you over the phone due to confidentiality laws.
Due to the high demand for bulk billing appointments and the high number of non-attendances and cancellations for GP appointments, we will charge a fee of $60 per single consultation or $120 per double consult, for any appointment that is not attended or not cancelled 2 hours before the appointment time.
If this account is not paid, a letter of suspension of services and the outstanding account will be issued. This letter will see your access to all services suspended until accounts are paid. There may also be the possibility of permanent suspension from the practice at the doctor’s discretion for repeat offenders.
We have many wasted appointments every day and by introducing this policy we hope patients will consider cancelling their appointments so that others may access them if required.

Your medical information is confidential; however, we on occasion forward certain medical information to other medical organisations (i.e. Immunisation Register, Specialists). All information is handled according to the current State legislation. The Privacy Amendment Act and the Australia Privacy Principles. Should you require any further information, please ask to speak to our Practice Manager.
In order to maintain confidentiality and to avoid delays, it is our Practice Policy not to interrupt the Doctors during consultations, except in the event of a medical emergency. All patients are asked to make an appointment for non-urgent advice, repeat prescriptions, referrals and most test results.
Our medical centre complies with all confidentiality and privacy obligations . Please ask our reception to view our privacy policy at any time.
Management of Personal Health Information
This practice has strict guidelines on confidentiality , patient consent ,consent for a 3rd party to obtain information,transferring health information and access and security of personal health information. It adheres to the National Privacy Principles Act and the Health Records and Information Act.
Telephone and Electronic Communication
Electronic communication is available via email at reception@jupiterhealthlakelands.com.au.All personal email is stored on a program specific encrypted computer data base system. Any email or phone calls addressed to clinical staff will be forwarded to the appropriate physician.
